The game retains mostly the same gameplay mechanics as its predecessor albeit with several changes. The weapon wheel from Fall of Man returns. All weapons are upgradeable and become more powerful the more they are used. Open level design allows players to tackle scenarios based on their playstyle. The system of regenerative health has been dropped for a circular system of health which doesn’t automatically regenerate forcing players to play more carefully and look around the levels for Sym-Bacs, however the durability of the player against all attacks is much higher than previous games.  These changes apply to both the Campaign and Multiplayer. The weather effects have also been improved in Resistance 3.

In 1953, shortly after the death of Lt. Nathan Hale at the hands of Joseph Capelli, Hale’s body was recovered by SRPA in which Dr. Fyodor Malikov discovered a cure from Hale’s blood, which he then isolated it and created a vaccine to the Chimeran virus. Capelli is dishonorably discharged from SRPA for Hale’s death, despite having done it due to Hale having turned into a Chimera. Four years later, Capelli married Susan Farley, the the adoptive-sister of Nathan Hale, and now lives in a community of survivors in Haven, Oklahoma. By this time, the Chimera had essentially conquered Earth and roughly 90% of humanity had been killed or converted. Those left had resorted to hiding under the ruins, trying to survive in a bleak world dominated by the Chimera, who are intent to wipe out the last remnants of the human race.
